St. Francis House (SFH) seeks a Rapid Rehousing Stabilization Case Manager to provide direct support to homeless individuals who are preparing for and transitioning to housing. Using a Housing First approach, the Case Manager will ensure that program participants are connected to all services and support they need to obtain and maintain permanent housing. This role works as a team with integrated services at SFH to stabilize individuals in housing, income, and behavioral health.

Since 1984, St. Francis House’s programs have served those experiencing homelessness and poverty. They offer a comprehensive array of services, all available under one roof, available 365 days of the year. They transform lives using a holistic approach to understand and address behavioral health, housing, and employment needs.

Major Responsibilities

  • Provide community-based stabilization services to a caseload of approximately 35 newly housed individuals with a history of homelessness
  • Support individuals with complex mental health, addiction, or medical needs
  • Proactively develop positive relationships with all individuals on the caseload
  • Conduct assessments and build collaborative service plans with residents that prioritize areas directly connected to improving stability in housing, income, and behavioral health
  • Provide linkages and referrals to community providers to support housing stabilization
  • Conduct regular home visits and outreach to keep participants engaged in services
  • Work with participants to develop and/or strengthen daily living skills including budgeting, employment/income, food, nutrition, and good tenancy practices
  • Monitor participants’ tenancy to identify concerns early, including landlord disputes, rent arrearages, and lease/code violations
  • Serve as a liaison with participants’ landlords or property managers
  • Maintain accurate records of work with participants and up-to-date tenant records in all required databases; regularly review caseload activities and goals

Preferred Qualifications

  • Case management experience in a direct care setting serving vulnerable populations
  • Willingness to work in the field minimum of 60% of the time
  • Experience working directly with people experiencing or who have recently exited homelessness strongly preferred
  • Lived experience of homelessness and/or recovery a plus
  • Bilingual in Spanish a plus
  • Bachelor’s degree preferred or high school diploma w/5+ years of relevant work experience
  • Ability to commute to Boston; required to travel via reliable personal vehicle to clients’ homes in Greater Boston/Eastern MA area
